APPLICANT REQUESTS THAT:

His DD Form 214, Certificate of Release or Discharge from Active 
Duty, Block 27, Reentry Code – “4J” be corrected to reflect 
voluntary separation and allow reenlistment into the Air Force 
Reserve or National Guard.


APPLICANT CONTENDS THAT:

His RE code is incorrect.  He was on the weight management 
program for a short time and that fact does not reflect his 
service to the Air Force.  He was not on the program when he 
separated.  He voluntarily separated at the end of his service 
contract.  This is confirmed by the narrative reason for 
separation listed on his DD Form 214 “Completion of Active 
Duty”.  

He did not understand the definition of these codes; otherwise, 
he would have challenged the code when he separated.  He would 
now like to join the Air Force Reserve or National Guard; 
however, this code is preventing this action.  He lives in the 
Washington, DC area and would like to go before the Board if it 
will help his case.

In support of his request, the applicant submitted a copy of his 
DD Form 214.

Exhibit A.


STATEMENT OF FACTS:

The applicant initially entered the Regular Air Force on 29 May 
97.

On 31 Jan 03, the applicant was furnished a Honorable discharge, 
and was credited with 5 years, 8 months, and 2 days of active 
service.   

The applicant’s DD Form 214 reflects a Separation Code of “MBK”, 
RE code of “4J” and a Narrative Reason for Separation of 
“Completion of Active Service”.

The remaining relevant facts pertaining to this application are 
contained in the memorandums prepared by the Air Force offices 
of primary responsibility (OPR), which are attached at Exhibits 
C and D.    


AIR FORCE EVALUATION:

AFPC/DPSIM recommends denial indicating there is no evidence of 
an error or an injustice.  

There is insufficient documentation to validate the applicant’s 
weight management program status at the time of separation.  In 
the applicant’s records is an AF Form 31, Airman Request for 
Early Separation/Separation Based on Change in Service 
Obligation, dated 19 Dec 02; item titled “Reason for Request” 
reflects “Early Release to Further Education”.  The AF Form 31, 
also shows the applicant’s request for separation was approved 
by the Commander, Military Personnel Flight.

The applicant provided very little evidence to indicate that the 
“4J” RE code was entered in error.  The applicant also did not 
provide enough evidence to substantiate his claim that he was 
not in the weight management program.

AFPC/DPSOA defers to AFPC/DPSIM, stating the applicant’s RE code 
indicates he was on the weight management program at the time of 
separation.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

Copies of the Air Force evaluations were forwarded to the 
applicant on 9 Jan 15 for review and comment within 30 days 
(Exhibit E).  As of this date, no response has been received by 
this office.


THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by 
existing law or regulations.

2.  The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file.

3.  Sufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice.  After a 
thorough review of the evidence of record and the applicant’s 
complete submission, we believe the applicant is the victim of 
an error or injustice.  While we note the comments of AFPC/DPSIM 
and AFPC/DPSOA indicating that relief should be denied because 
the applicant’s records contain no documentation substantiating 
the applicant’s contention that was not on the weight management 
program, we believe a preponderance of the evidence 
substantiates that corrective action is warranted.  In this 
respect, we note the applicant’s master personnel record 
includes an AF Form 31, Airman Request for Early Separation / 
Separation Based on Change in Service Obligation, in which he 
requested to separate early to further his education.  
Therefore, we recommend the applicant’s records be corrected as 
indicated below.  


THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air 
Force relating to the APPLICANT be corrected to show that his DD 
Form 214, Certificate of Release or Discharge from Active Duty, 
Item 27, Reentry Code, be corrected to reflect “3K”.
